OV vs Hsopital Observation

How would this be billed sinced the OV have been billed and paid for?

OV: 12/16/14(example) billed/paid for by insurance
Obs: 12/16/14-12/18/14

There should be time difference between OV and Obs.
For example, OV is done in the morning, then patient come again to the department with deteriorate condition or with different medical condition and physician decided to place the patient in Obs,then it can be billed as initial visit, subsequent visit and obs discharge for the above mentioned dates.
