Outpatient surgery, same day hospital transfer

Patient had outpatient surgery: UPP '42145', tonsils '42826', and trach '31600' (added due to breathing difficulty). Then was transferred to another hospital the same day due to respiratory problems. The second hospital was inpatient admission, and the patient was seen daily for three days '99232'. How do I code the admission to the second hospital on the same day as the initial outpatient surgery? Would the daily visits need a modifier? Can I also charge for hospital discharge?
Additional problem: this patient was taken back the the first hospital, the day after inpatient discharge, for a trach change. Is the correct coding for this surgery '31502' with a '78' modifier? Please help!!!
