Hello, A patient had a Vaginal Hysterectomy on 10/17th scheduled as an outpatient but on the same visit while recovering had a seizure and was admitted to ICU (IN) on 10/18th. My question is what status do I code the Hysterectomy OT? IF not on the claim I would have to add the admit date and it would be a discrepancy on the claim admitted on the 18th and procedure on the 17th?? Please let me know.

There will be two admission dates for two seperate dates (two seperate forms) with different diagnosis. One will be with diagnosis code of hysterectomy for which patient was admitted (outpatient) on 10/17. Second will be diagnosis code of seizure for which patient was admitted to ICU (IN) on 10/18th. For further clarity please send some more details, so that we can check with facility and POS (place of service) as well. Thank you.
