I have a new client who still thinks we can use pre 2015 cpt codes. I have explained that a lot of the codes have been bundled and he wants to unbundle them anyway. Could you help in coding the procedures below?

1-caudal epidural steroid injection
2-lumbar sine epidurogram
3-left sacroiliac joint injection
4-left sacroiliac joint arthrogram
5-injection for Myelogram dye for interpretation
6-use of fluroscopy

Help code the following
1-caudal epidural steroid injection
2-lumbar sine epidurogram
3- facet joint arthrogram right L4-L5 and L5-S1
4- facet joint injection right L4-L5 and L5-S1
5-injection for Myelogram dye for interpretation
6-needle localization under fluroscopy

What if scenario 2 added Right hip greater trocahnteric injection? what would be the correct code sequence at that point?

If done on at the same time we do not bill separate?

You can bill 72275 along with 62310/62311. It does include flouro, as this is the device taking the image. Just like flouro it is billed per region, not level. As mentioned above you would need a separate radiology report to support this code. The same modifier guidelines and principles apply to 72275 that apply to almost all other radiology codes (i.e., modifier -26 if you don't own the equipment, etc.). Hope this helps!
