Dr. did a ORIF metacarpal fracture using an IM metacarpal nail.
A photoscopic C-arm unit was used for radiograpic imaging through the procedure. The IM metacrpal nail trocar was then inserted manually through the bone into the intramedullary canal. The IM nail was then advanced through the trocar into the intramedullary canal of the base of the 4th metacarpal. Under C-arm guidance the fracture was reduced and the IM nail was advanced down the intramedullary can of the bone.

What CPT code would be appropriate. Can only find CPT 26615 but does not mention the use of a IM nail. Any help would be appreciated. THanks.

Open fracture care takes place when the physician surgically opens the fractured site and internal fixation takes place, or the physician remotely exposes the fractured bone to allow insertion of an intermedullary nail. The physician makes an incision over the fracture site and the fracture is manipulated into the correct position. The surgeon approaches the area proximal or distal to the fracture site and threads a nail down into the intramedullary canal, through the bone, and across the fracture site. Many times, interlocking screws are placed at angles to the nail to further stabilize the fracture.
