Hello, we recently converted over from using IV Diazepam to Oral Diazepam and i cannot seem to find a appropriate code for the oral diazepam, the code for IV Diazepam we were using is (j3360). So my question is can you please help me out with finding an appropriate code for the oral diazepam? The only code i can find is (j3490)unclassified drugs will this work? or is there a more specified one?
Thanks in advance for you help on this

No, you are right. There is no new HCPCS for oral diazepam (valium). You are right to choose J3490 for unclassified drugs. Still giving you link for current 2012 drug table from CMS:
