Are eye patches " A6412" considered incident to the service or can they be billed separately?

A6412 Eye patch if "incident to" a physician's service (not separately payable) or if supply for implanted prosthetic device or implanted DME.

The eye patch (A6412), payers generally don't cover the item. Please enquire about it from your respective payor.
