it looks like a dense adhesions simply tore the common duct. I selected a 14 French T-tube. I cut away the back portion of the intraluminal tube so that it was basically a flat surface rather than a tube. I placed the tube with the ductal structure and i used the 4-0 Vicryl sutures on an RB1 needle to suture both ends. The tube almost completely closed the defect and it only took 2 sutures to make it basically watertight. I obtained a colangiogram and this time it looked like a straightforward T-tube.
