Open Chole and cholecystocolonic fistula

The surgeon wrote: open cholecystectomy with takedown of the cholecysto-colonic fistula, primary closure of the fistula present in the colon with double layer closure with massive intra-abdominal irrigation. We coded 47600 for the open chole, 44650 for the fistula and 49020 for the massive irrigation of the spilled abscess.
Insurance request op-note and consequently denied all but 47600. Are they right?

As, I have seen all the CPT codes are valid code for the scenario given. May be they have some confusion in these services, so please provide them op-note as they required for this claim or you can query them regarding denial. Coding part is correct.
