Office visits that include therapy

Any suggestions on how to bill a patinet for both and office vists and then therapy treatments given during that visit? Example: diag: 724.4, 715.11, 722.10 for therapy and 401.1, 443.9, 285.9 for office visit. CPT's are 97039, 97010, 97032, 96372, 99213.

In this case, you can bill these CPT codes together, but you need to append modifier 25 with office visit(99213).
