Office visit and injection on same day

If a patient comes in to our clinic with pneumonia and the dr does a complete exam and decides to give this patient a shot of Rocephin. Is it correct to bill the e/m for office visit (with 25 modifier) and the injection fee '96372' along with the medicine?

This is how it should be billed. But please note:

(Do not report 96372 for injections given without direct physician or other qualified health care professional supervision. To report, use 99211. Hospitals may report 96372 when the physician or other qualified health care professional is not present)
