How do you code the following for Medicare 2 midnight rule:

Patient is seen in ER and admitted to observation on 4/1/2014 at 1:00pm. Patient is also seen as observation patient on 4/2/2014 at 1:00pm. Patient is admitted on 4/3/2014 after two midnights. Patient has subsequent visits on dates 4/4/2014 and 4/5/2014. Patient discharged on 4/6/2014.
