Observation to inpatient

Today i seen patient initially admit to inpatient and the next day order for observation admission. how to code in this situation?

For today's service, you should report code from series 99221-99223 (Initial Hospital Inpatient Care Services).


For next day's service, you should report code from series 99224-99226 (Subsequent Hospital Observation Care Services).

once patient admit to inpatient why the patient come back to observation again. if the conditions are stable they will discharge the patient from inpatient. is there any purpose or guideline for inpatient to observation admit.

You are correct. We also have never seen such a situation where a patient has been admitted to observation from hospital inpatient. There is no guideline regarding this. There may be some error in the documentation. We advise you to contact the doctor's office to clarify this and get it corrected if it is not correct.
