EFW 192 grams, 59th percentile.

Mean gestational age by today's u/s 17 weeks, 3 days +_2 weeks. Estimated date of delivery by today's u/s 9-27-11.

Is this enough documentation to bill out a complete u/s 76801 or should it be limited 76815.

Here gestational age is 17 weeks. So, the code should be 76815 as being limited for limited documentation. 76801 is applicable only if less than 14 weeks.
