Can a x-ray tech bill 99211 under doctor when he is in the pain clinic in the following scenario. The patient is questioned regarding the efficacy, use of the controlled substance and current pills are counted to verify correct usage. The information is reviewed by provider, and if needed, writes another RX. Our MA, LPN and RN also perform this service. Does this type visit need to be documented/ordered in the patient's chart ahead of the visit?

Yes, x-ray tech bill 99211 under doctor. As per general coding guidelines for 99211, the provider, often a nurse, spends an average of 5 minutes face to face with an established patient. This service does not require a physician or other qualified healthcare provider, such as a physician assistant, to see the patient. The presenting problem is typically minimal.

The second question of the initial post was not addressed. Does this type visit need to be documented/ordered in the patient's chart ahead of the visit (set up by the provider)?
