Nonunion

How would I code the following:
DIAGNOSIS:
1.Calcaneocuboid nonunion, left.
2.Failed posterior tib tendon reconstruction, left.

PROCEDURE:
1.Revision of calcaneocuboid nonunion, left.
2.Talonavicular fusion, left.
3.Subtalar fusion, left.
4.Harvest of proximal tib autograft, left.
5.Removal of lateral calcaneal hardware necessary to perform the case, left.
6.Removal of medial midfoot hardware necessary to perform the case, left.

I know 28725 and 28740 but am not sure about the calcaneocuboid nonunion.

Failed posterior tib tendon reconstruction: T84.498A
Procedure:
Used CPT 28725, 28740 and 28899 codes to code this scenario.

Unlisted CPT code 28899 is used for revision of calcaneocuboid nonunion and removal of lateral and midfoot hardware.
