WE CANNOT BILL FOR CLOSED TREATMENT WITHOUT MANIPULATION OF MULTIPLE FRACTURES, SUCH AS METATARSAL FRACTURES WITH CMS. FOR EXAMPLE IF WE HAVE THREE METATARSAL FRACTURES TREATED IN A CLOSED FASHION WE ARE ONLY ALLOWED TO REPORT ONE. DOES THIS APPLY IF YOU HAVE A METATARSAL FX AND A CUNEIFORM FX OR ANOTHER EXAMPLE IF YOU HAVE A METACARPAL FX AND A PHALANX FX ALL CLOSED TREATMENT WITHOUT MANIPULATION.

Hi, One CPT code for closed fracture treatment without manipulation is reported when the multiple fractures with a single cast, strapping, or splint. Same applies to any combination of closed fractures treatment without manipulation in addition to at least one fracture requiring manipulation. Only one CPT code for closed treatment with manipulation is reported. If no cast, strapping, or splint is required, report only one CPT code for closed dislocation or fracture treatment for the anatomic area that would have been treated by a single cast, strap, or splint. Only one unit of service is reported for closed treatment without manipulation of fractures of multiple similar bones, whether or not a cast, splint, or strap is applied. The same is for multiple carpal, tarsal, or metatarsal bones in one extremity. One code would be reported for closed treatment without manipulation of multiple fractured metacarpal bones in one hand.
