no laminectomy

Diane Posted Wed 22nd of June, 2016 16:48:03 PM
this patient was status post decompressive laminectomies at L3-L5 by another doctor. We went in and did a revision of spine surgery with external neurolysis of L3, L4 and L5 nerve roots bilaterally, with foraminotomies bilateral at L3-L4, L4-L5 and L5-S1 with excision of massive herniated disc and fragments at L4-5 on rt with bilateral discectomies at L5-S1. Since laminectomies had previously been done we did not do laminectomies, all the codes state laminectomy. So do I just use the laminectomy code 63042 for re-exploration and use a -22 modifier for reduced procedure? Can I also use the code 64714 lumbar plexus for the external neurolysis. I do not want the fee to be drastically reduce because I use a modifier 22 because there was still alot of work done. Please advise on coding. Thank you

Hi, Use Modifier 52 reduce services instead of modifier 22. To bill Cpt 64714 modifier 59 is applicable with cpt 63042.
