Hi Louise,

Thanks for your question.

Due to limited documentation, please find below the suggested codes you may choose. Please corelate the codes based on medical record.

Closure device placement at the vascular access site is inherent to the catheterization procedure and not separately reportable. So Angio-Seal is not billable separately.

36245 - Selective catheter placement, arterial system; each first order abdominal, pelvic, or lower extremity artery branch, within a vascular family

36200 - Introduction of catheter, aorta

+76937-Ultrasound guidance for vascular access requiring ultrasound evaluation of potential access sites, documentation of selected vessel patency, concurrent realtime ultrasound visualization of vascular needle entry, with permanent recording and reporting (List separately in addition to code for primary procedure)

37221-Revascularization, endovascular, open or percutaneous, iliac artery, unilateral, initial vessel; with transluminal stent placement(s), includes angioplasty within the same vessel, when performed

Please feel free to write if you need further assistance.

Thanks

Using ultrasound guidance 5 Fr sheath was placed in R CFA. Sheath angiogram showed patent R CFA,DFA and SFA proximally and proper access placement. Then using ultrasound guidance 5 Fr sheath was placed in L CFA. Sheath angiogram showed patent L CFA,DFA and SFA proximally and proper access placement. heparinized with an Omni cathter which was advanced from the left side to infra-renal aorta. Aortogram was performed : Patent infrarenal aorta; on right, stenosis of CIA, patent external iliac artery (EIA), internal iliac artery (IIA); on left, stenosis of CIA, patent EIA, IIA. We up-sized the sheath to 7 Fr and advanced Advantage wires in both CIAs and using road map and kissing angioplasty technique, deployed a stent on the right CIA and stent on the left CIA. The right side was post dilated with Mustang balloon.
