The surgeon removes two epidermal inclusion cysts from the patient's left and right mandible area i.e. 5.5x4.5 cm and 3x3 cm, both of which require intermediate repair).

Because both repairs on the mandible are of the same type (intermediate) add the lengths together (5.5 + 3 = 8.5 cm) to select repair code. The combined size for both cysts will be 8.5 cm, so the appropriate CPT code for intermediate repair will be 12054 (Repair, intermediate, wounds of face, ears, eyelids, nose, lips and/or mucous membranes; 7.6 cm to 12.5 cm).
