Nerve decompression and listing of modifiers

Kerry Posted Tue 03rd of November, 2015 20:22:41 PM

I amhaving problems with coding nerve decompression (64708) and the listing of the modifiers. I am confused as to what the correct modifier for the same code done on separate nerves, do I use 76 or 59 (XS). I also have modifiers 79, GA, RT and if there is an assist an 80 modifier to add the claim as well. So we would use 99 modifier in the assistant situation. Any guidance would be great.!!

Thanks for your question.

If the same procedure done on two separate nerves of the same site, modifier 59 (XS) is appropriate with CPT and if the same procedure done on the other site, modifier RT or LT is appropriate to use.
If the assistant surgeon is there, use modifier 80 with CPT code.

When more than four modifiers apply, enter modifier 99 in the first modifier field. In the narrative field (item 19 on the claim form) list all modifiers in the correct ranking order being sure to identify which detail line or procedure code to which the modifiers apply.
