Negative path report

To use CPT codes 52234 - 52240 does the pathology report have to be positive for malignancy? What if the path report states chronic cystitis, cystitis cystica, etc?

In general, a tumor means any abnormal growth, which may or may not be malignant. It seems assigning any of the CPT codes 52234 – 52240, is based on the size of tumor, and not the pathology. However to answer your question accurately, I need to have more information of the case. Thanks!!
