WHEN A PT IS SEEN FOR COUGH AND WHEEZING AND PROVIDER GAVE A NEBULIZER TREATMENT FOR THE WHEEZING AND I APPEND MODIFIER 25 TO THE OFFICE VISIT BECAUSE OF THE NEBULIZER TREATMENT.PLEASE I NEED YOUR EXPECT CLARITY. THANK YOU.

Thanks for your question. You may add modifier 25 to the E/M service to indicate that a significant, separately identifiable E/M service has been provided by the same physician/qualified NPP to the same patient on the same day as another procedure or other service with a global fee period. According to CCI, the E/M service is a column two code to the breathing treatment. Your documentation should be able to support the use of the modifier in this case. Please see that your documentation can support that the treatment was not planned, was related or unrelated to the office visit and that the office visit's documentation can stand alone.
