Nail removal code

Please advise correct code(s) for the following procedure: The patient was brought to minor operating suite and placed on the operating table in supine position. The upper extremity was prepped and draped in usual sterile fashion. After time out procedure, review of the the consent form, and laterality confirmation was performed, digital block was administered to the 1st digit. 1% lidocaine with epinephrine was administered for a total of 5 cc using a 27-gauge needle. The ulnar 1/3 of the nail plate was removed. Caseous material from the recesses of the germinal matrix were sent for AFP and fungal cultures. I created a crescenteric incision at the ulnar base of the nail plate. The skin and subcutaneous tissue were excised to the level of the germinal matrix, but the roof of the matrix was left intact. I swabbed this area for culture and gram stain. The area was dressed with antibiotic ointment and xeroform and a bulky compressive dressing was applied. There were no complications.

You would use 11750 (excision of nail and nail matrix, partial or complete) for this procedure.
