Modifier indicators "1" & "0"

The issue with E/M denials and vaccinations with their respective administrative codes have a complicated turn, or it may be something so simple that it cannot be viewed as such.
Although Modifier 25 is been used in E/M codes above 99211 and all the Child Check Up codes, the E/M stills gets denied. Now I'm reading about Modifiers indicators 1 & 2, not much after that. Can you enlighten on this? Thanks!

Can you please tell what is your specific question? For instance, is there a particular code combination that is problematic? I would be more helpful if you provide me specificity?
