modifier -25 with injections?

When we see a patient for annual preventive visit and also a problem visit we add -25 to the problem visit and usually are paid for both. If patient also has an injection that day insurance is bundling the preventive visit with the injection charge. Is it appropriate to use -25 on both the preventive visit and the office visit?

If a patient encountered for condition other than the preventive services, and the injection was performed pertaining to that particular condition; it can be billed separately. For example- if patient is diagnosed with OA knee and injection of kenalog is administered on the same DOS, then an appropriate injection code along with appropriate E/M service can be billed together.
