Medicare Edit/Rejection - can someone help?

We have a Medicare electronic rejection stating "diagnosis code invalid for date of service (DOS within date range 20111001 and 20120930). We billed for a e&m service with diagnoses 518.389,794.2, 162.9 and 698.9. The edit doesn't specify which diagnosis, but I can't find anything wrong with any of them (no missing digits or extra digits). Can someone give me some ideas?
