Lumbr Transforaminal

You answered a question a few minutes ago from a procedure note I sent you today regarding transforaminal levels. Based on the procedure note would you consider levels L4 and L5 nerve blocks a single level nerve block,64483? I need clarificaton on this.

See these are 2 different levels:

L4-L5 transforaminal epidural steroid injection (L5 nerve root) and L5-S1 transforaminal epidural steroid injection (L5 nerve root)

Levels L4 and L5 nerve blocks represent two different levels.You are going to use both the 64483 and 64484 it has to be injected into each nerve. These are L4-5 and L5-S1 transforaminal injections into epidural space around nerve root L4 and L5 respectively.
