If the provider performed an extension of previous fusion of L2-3 the following way: Incision made and bilateral subperiosteal dissection was performed. laminectomy/facetectomy/foraminotomy on left side, Thecal sac was thoroughly decompressed, PEEK structural allograft was sized, packed with allograft along with autograft and packed into the interspace expanded to appropriate height and then an interspinous plate was then placed. Would the codes be 22633/22840/22853/20931/77003?

Codes 22633, 22840, 22853, and 20931 are correct. You need to make sure from the medical record if 77003 (Fluoroscopic guidance…) was also performed. Moreover, for autograft, you should choose a code from 20936-20938.
