Is the given scenario for long term use of a particular drug or patient is currently having more then 25 different drugs? Or is it a kind of overdose (25 or more number of non-prescribe drug is taken)?
These three are totally different circumstances. We generally use V58.6X to report the long term medication for the associated chronic problem.
