Is there a resource for how to document in the medical record cosmetic lip filler injections performed in the office?

Code ranges 11950-11954 appears to be most closest to the dermal filler injections for non-CMS payers. But, the payer policy should be checked, whether they allow costmetic surgery claims for lip filler injections. Medical necessity should also be mentioned for the procedure.

Code 11950 medical documentation suggests that the provider injects the required amount of natural or synthetic filler, such as collagen, into the subcutaneous layer. The material fills skin depressions due to scars and/or wrinkles. Use this code for an injection that is 1 cc or less.
