LHC but no LV, FFR and OCT then PCI

Carl Posted Wed 10th of February, 2016 16:08:36 PM

Left heart catheterization.
Coronary angiography.
Optical Coherence Tomography (OCT) in the LAD.
Fractional Flow Reserve (FFR) in the LAD and RCA.
Coronary angioplasty and stent in the RCA.

Patient presents for evaluation of chest pain and dynamic EKG changes.
No prior cardiac history. He has no CHF.
He underwent coronary angiography with left heart catheterization and coronary angiography.
Left ventricular pressure was measured but left ventriculography was not done.
Significant atherosclerosis of the LAD and RCA were noted.
He underwent FFR measurement followed by OCT in the LAD. No angioplasty needed in the LAD.
He then underwent FFR in the RCA, followed by coronary angioplasty with stent in the RCA.
I was proposed two CPT Codes sequences:
CPT Codes: 93458-59, 93571-LD, 93572-RC, 92928-RC, 0291T-LD
or CPT Codes: 92928-RC, 93458-26-XU, 93571-26, 0291T

Which is the more appropriate CPT Codes sequences.

Thanks for the Query

I would suggest appropriate codes sequence for the given scenario are 92928-RC, 93458-XU-26, 93571-26, 93572-26 and 0291T.
