How do you calculate the dimensions of the area? phys captured cpt code 11426 for the neck. Is this correct? Then the markings for the lower back excision were made, incorporating a 1 mm margin of apparently uninvolved skin. The dimensions of the area of excision were 3.0 cm x 1.8 cm.   Then the markings for posterior neck excision were made, incorporating a 1 mm margin of apparently uninvolved skin. The dimensions of the area of excision were 1.5 cm x 0.4 cm.   Then the markings for anterior left neck excision were made, incorporating a 1 mm margin of apparently uninvolved skin. The dimensions of the area of excision were 1.5 cm x 1.5 cm.   Then the markings for right fourth toe excision were made, incorporating a 1 mm margin of apparently uninvolved skin. The dimensions of the area of excision were 1.5 cm x 2.0 cm.

Hi,

CPT describes the total excised lesion size as the greatest clinical diameter of the apparent lesion plus that margin required for complete excision (lesion diameter plus the most narrow margins required equals the excised diameter). For instance, if an OMS removes a benign lesion from the skin of a patient's face measuring 2.0 cm at its widest point with margins of 0.3 cm removed on each side, the total excised diameter would be 2.6 cm (2.0 cm + 0.3 cm + 0.3 cm = 2.6 cm) reportable with CPT code 11443.

As per your documentation,

  • Lower back excision : 3*1.8cm and 1mm margin= 3+(2*0.1)= 3.2cm= CPT code 11404
  • Post. neck excision: 1.5*04 and 1mm margin= 1.5+(2*0.1)= 1.7cm= CPT code 11422
  • Left neck excision:1.5*1.5 and 1mm margin= 1.5+(2*0.1)= 1.7cm= CPT code 11422
  • Toe excision: 1.5*2 and 1mm margin= 2+(2*0.1)=2.2cm= CPT code 11423
