Leg Laceration

How would I code the following:
DIAGNOSIS:
1.Partial musculotendinous junction laceration of the peroneal longus.
2.Partial laceration of the superficial branch of the peroneal nerve.

PROCEDURE:
1.Exploration of the wound with repair of the peroneal longus at the musculotendinous junction.
2.Repair of superficial peroneal nerve and closure of laceration.

Use CPT code 28200 for repair of Peroneal longus (at foot level) or CPT code 27658 for repair of Peroneal longus at leg level. Use CPT 64708 for repair of superficial peroneal nerve at leg level or CPT code 64704 for repair of superficial peroneal nerve at foot level.
