Please answer the following coding scenario:

My doctor has performed "Left Heart Catheterization with LV, Coronary Angiogram with Coronary Stent Placement". In this situation we are reporting 92928 & 93458-59. Is it correct? or is 93458 included in 92928 per CCI edits? Please clarify and suggest us the correct coding. Patient do not have any Prior coronary angiogram in the recent past.

According to CCI edits, CPT code 93458-59 can be billed with CPT 92928, if Left Heart Catheterization with or without LV is medically reasonable and diagnostically necessary. and it precedes the percutaneous coronary artery intervention.
