I am inclining towards an unlisted code here. But let me check with my CE over this for a definitive answer.

If this procedure was performed in which direct visualization of the disc and nerve root with the naked eye could be performed, then 63056 and 69990 would be the appropriate codes to report. If direct visualization was not possible (meaning an endoscopic or percutaneous discectomy was performed), then 62287 would be the appropriate code to report.
