The surgeon performed a "right L3-L4 lateral transforaminal discectomy with microsurgical technique. spinal needle was inserted and x-ray obtained for initial localization

There is insufficient info provided to make a definitive determination. If this procedure was performed in which direct visualization of the disc and nerve root with the naked eye could be performed, then 63056 and 69990 would be the appropriate codes to report. If direct visualization was not possible (meaning an endoscopic or percutaneous discectomy was performed), then 62287 would be the appropriate code to report.
