Laparoscopy w/ Lysis of adhesions; excision of right partubal cysts
hysteroscopy with morcellation of endometrial polyp.
DX: Pelvic adhesive disease
Paratubal cysts
dysmenorrhea
Endometrial Polyps

CPT code:
1. Laparoscopy w/ Lysis of adhesions = 58660
2. excision of right partubal cysts = 58662
3. hysteroscopy with morcellation of endometrial polyp = 58558

CCI edits states that "Code 58660 is a column 2 code for 58662, These codes cannot be billed together in any circumstances."

Bill CPT 58662 and 58558 only.

DX: Pelvic adhesive disease = N73.6
Paratubal cysts = N83.8
dysmenorrhea = N94.6
Endometrial Polyps = N84.0

Would I use 58662 even though cyst was not on the ovary but was a para tubal cyst?

Yes, CPT code 58662 can be used for lesions of the ovary, pelvic viscera, or peritoneal surface. And Para tubal cyst comes under pelvic viscera.
