Laparoscopy right salpingo-ooporectomy w/ Lysis of adhesions and ablation of end

My physician did an Operative Laparoscopy, right salpingo-oophorectomy, lysis of adhesions and Ablation of endometriosis.
Dx: 16 Cm Right ovarian dermoid and Stage 1 endometriosis
What Cpt code would I use.
My Doctor wants me to bill with a modifier 22 for the lysis of adhesions and a 59 modifier for the endometriosis?
Am I suppose to bill 58661 and 58662?

Thanks for your question.

The most appropriate code is 58661 and we will not use modifier 22 for the lysis of adhesions because it is inclusive with right salpingo-oophorectomy. There is no specific code for laparoscopic endometrial ablation, so to code this use unlisted code 58679 with modifier 51.

We did ablation of endometriosis? Is that still the same code 58679?

Hi,
I apology for wrong code suggestion.
If the ablation of endometriosis is laparoscopic procedure, we will CPT code 58578 (Unlisted Laparoscopic procedure, uterus).
If it is done hysteroscopically then use 58563 (Hysteroscopy, surgical; with endometrial ablation)
