Laparoscopy assisted Vag Hysterectomy

Good Morning, I need some assistance with the coding of a surgery. The surgeon performed a Laparoscoy assisted Vaginal hysterectomy, lysis of adhesion and left salpingo-oophorectomy and cystoscopy. I want to make sure I have not missed and codes to be billed.

You may report one of the following codes depending upon the weight of the uterus. If the weight of uterus is less than 250 g, then report code 58552 (Laparoscopy, Surgical, With Vaginal Hysterectomy, For Uterus 250 G Or Less; With Removal Of Tube(S) And/Or Ovary(S)). If weight of uterus is more than 250 g, report code 58554 (Laparoscopy, Surgical, With Vaginal Hysterectomy, For Uterus Greater Than 250 G; With Removal Of Tube(S) And/Or Ovary(S)).


Lysis of adhesion, code 58660 (Laparoscopy, Surgical; With Lysis Of Adhesions (Salpingolysis, Ovariolysis) (Separate Procedure)), should not be reported with the above codes because of CCI Edits.


Cystoscopy code 52000 (Cystourethroscopy (separate procedure)) is a separate procedure and should not be reported with any of the above procedures.
