Septum resection
Polypectomy
Operative Laparoscopy
Chromotubation
Ablation of endometriosis
Lysis of adhesions
Aspiration of right ovarian endometrioma
Ablation of endometriosis
How do I bill for all these?

Appropriate CPT codes as per documentation:

Hysteroscopy with Septum resection: Use code 58560
Laparoscopy with Chromotubation: Use code 58350
Ablation of endometriosis: Use code 58563

Code 58563 has a CCI conflict with code 58558. A modifier is not allowed to override this relationship.So do not use CPT code 58558 separately for polypectomy.

Lysis of adhesions: Use code 58559
Aspiration of right ovarian endometrioma: Use code 49322

The Ablation of endometriosis and Lysis of adhesions and aspiration of right ovarian endometrioma was done through an operative Laparoscopy, Do I still use 49322 or 58662?

Yes, we will still go for 49322, as this code says aspiration of ovarian cyst. ovarian endometrioma is a benign, estrogen-dependent cyst.
Where as cpt 58662, is for fulgaration or laparoscopic electrical cautery destruction of an ovarian, pelvic or peritoneal lesion.
