We use laparoscopic grasping instruments and bipolar cutter to take down the ovarian structure on the right, dissected out the adhesions and visualize the left adnexal region showing a normal left ovary and portion of fallopian tube. Once the adnexal cyst was removed, all the trocars were removed. DX: Benign vascularized connective tissue, negative for inflammatory or neoplastic change.
Findings: Right adnexal area with a cystic structure which was removed. What CPT code should I Use and Dx ICD9:? I originally used the 58661 but 2nd procedure beginning done and her ovaries being removed.

CPT code 58661 seems to be appropriate.

Patient had second surgery and my provider removed left ovary and his assisted surgeon with removal of right ovary. How would I code this? I don' think I can use the 58661 again. ??

Please tell when the first surgery was performed, also global period for CPT code58661 is 10.
There is an appropriate modifier '80' for assistant surgeon.

First surgery 4/17/15 58661 dx 620.2
2nd surgery 1/15/16
My provider OB/GYN did the LSO and he assisted another surgeon(not GY) who did a RSO with adhesion removal of the bowel.
I am not sure how to code. Can I use the 58661 again for the 1/15/16 surgery.

Yes you can use CPT code 58661 again, as the global day period of the surgery is 10 days only and the second surgery was performed beyond the global period.
