Laparascopic repair of a cholecystoduodenal fistula

Cheryl Posted Tue 26th of March, 2019 14:37:06 PM
My doctor did a laparascopic cholecystectomy with intraopertive cholagiogram (47563) as well as laparascopic takedown and repair of a cholecystoduodenal fistula. What CPT code would be used for this procedure?

Hi Cheryl,

There is no exact code for repair of a cholecystoduodenal fistula, which is done laparoscopically. So you'll need to use the unlisted laparoscopic code 44238 (Unlisted laparoscopy procedure, intestine [except rectum]) instead. Also, you should include a cover letter stating why you are using the unlisted procedure code. This separate report should explain, in simple, straightforward language, exactly what the physician did.

NOTE: CPT® provides a code for fistulae closure 44650 (Closure of enteroenteric or enterocolic fistula). But you can't use the code for this case, because it's an open code, and your case is a laparoscopic procedure.
