Lab CPT CODE 87480

We have billed 87480,87510,87760 and used ICD10 N77.1. Insurance is denying the 87480 for the N77.1 are we using the correct CPT codes and is there a different dx we should be using

Icd N77.1 is not payable with Cpt code 87480. kindly provide the documentation on the basis of which N77.1 and 87480 has been coded.

Patient had vaginal discharge and under assessment:dx vaginitis,vulvitis and vulvovaginitis. Plan: provider has ordered AFFIRM for vaginitis, vulvitis, vulvovaginitis

Kindly confirm if Infectious agent detection by nucleic acid (DNA or RNA); Candida species, direct probe technique was performed for the billed diagnosis. As per assessment Icd code N77.1 is correct for diagnosis vaginitis, vuluvitis and vulvovaginitis.
Hence we need to confirm the CPT code.
