J3301 NDC

CPT J3301 denied as invalid NDC Code. We billed with NDC#00003029328. Claims DOS are between May and July this year. The NDC on the kenalog 40 mg bottle is 0003029328 but we added a zero at the beginning because the rule it says it has to be 11 digits in 5-4-2 digit sections. if a digit is missing we need to add a 0 at the beginning of that section that's why we are entering the above mentioned 11 digit . Do you know if there is any changes about which NDC to use to bill for this code? By the way since we have to bill 40 mg and each unit represents 10 mg we are billing 4 units which we haven't had any denial for that. The reason for denial is NDC number
