J3301 & J1094

Vera Posted Wed 29th of October, 2014 13:34:43 PM

We abruptly stopped being paid for these codes quite some time ago and have made suggested changes by our software vendor to no avail.

For J3301, we provide this information: >NOC=Kenalog 40mg 00003029328> **CORRECTED CLAIM**

For J1094, we provide this information: >NOC=Dexamethasone Sod Phos 4mg/ml 63323016530>

Please provide information on how to correctly bill this since previous efforts have failed.
