I was trying to find out how to bill for depo shots? I am currently billing J1050 fp 96372 with v25.9 with Medicaid and I am getting a rejection code for the J1050 my system wouldn't send it out. I never get paid for the admin fee, thank you

The use of modifier U1 (Medicaid level of care 1, as defined by each state) may be required for used of J1050 for contraception.

The diagnosis of V25.49 (Surveillance of other contraceptive method. checking, reinsertion, or removal of contraceptive device, repeat prescription for contraceptive method or routine examination in connection with contraceptive maintenance)may also be assigned depending on documentation.
