The recommended dosage of aflibercept (eylea) is 2mg (0.05 mL) administered by intravitreal injection.

Since, the Intravitreal injection procedure was performed bilaterally, append modifier 50 (Bilateral Procedure) to CPT® code 67028.


Report the scenario as below:

67028-50 (to indicate that the procedure was performed bilaterally)

J0178 x 4 (2mg dosage per affected eye).
