The S codes are used by the Blue Cross/Blue Shield Association (BCBSA) and the Health Insurance Association of America (HIAA) to report drugs, services, and supplies for which there are no national codes but for which codes are needed by the private sector to implement policies, programs, or claims processing. They are for the purpose of meeting the particular needs of the private sector. These codes are also used by the Medicaid program, but they are not payable by Medicare

Use code S9501 for the Home infusion therapy, antibiotic, antiviral, or antifungal therapy; once every 12 hours; administrative services, professional pharmacy services, care coordination, and all necessary supplies and equipment (drugs and nursing visits coded separately), per diem.
As per your scenario Use 2 units of S9501x2.

Use Code S9373 Home infusion therapy, hydration therapy; administrative services, professional pharmacy services, care coordination, and all necessary supplies and equipment (drugs and nursing visits coded separately), per diem.
As per your scenario Use 2 units of S9501x2.

Use J codes for Vancomycin 1 gm BID, Cipro 40mg BID, Nexium 40mg OPD, Clexane 40mg OPD.

Use Code E0781 for pump rental.
Use code 99601 for Home infusion/specialty drug administration, per visit (up to 2 hours).
