An total abdominal hysterectomy was done, but before is was performed the doctor removed a 10 lb subserosal tumor. What is the best way to code this?

Removal of a subserosal tumor is considered part of the hysterectomy and you cannot bill it separately. You need to report only the hysterectomy code. The only exception is pre-hysterectomy removal of a myoma/pelvic tumor that is not attached to the uterus. You need to support this with proper documentation to justify the separate procedure.
