is the 719.4x "pain in ..." code always necessary?

we are struggling between our billing & coding departments to determine if the 719.4x joint pain code is always needed on o/v notes. i understood if coding a fracture, for instance, that the pain was inherent to the fracture and you would not code the wrist pain, only code the wrist fracture. our billing dept says medicare needs/wants pain codes on any charge where pt c/o "pain" even if we have more specific diagnosis for the pain. please help - thank you

As per guidelines, if this is an initial visit, we will code only fracture and if this is a followup visit for fracture and the chief complaint is pain, then we will code pain with fracture code.
